Physician Assistant (PA) or Nurse Practitioner (NP)

Hybrid Role – In Office + Remote

Service Area: North Phoenix (Anthem) – West Valley – Scottsdale

Alliance Medical Clinic and Ternion Physician Group are seeking a compassionate and motivated Physician Assistant (PA) or Nurse Practitioner (NP) to join our growing integrated care team.

This is a hybrid clinical role combining in-office care with remote and on-site visits to assisted living communities, focused on the management of dementia, chronic illness, and palliative care needs in geriatric populations.

Position Overview:

The PA or NP will provide comprehensive medical care to patients in:

• Outpatient office settings.
• Assisted Living Facilities.
• Memory Care Communities.
• Group Homes.
• Home-based or Telehealth visits (as appropriate).

This role includes a mix of in-person visits and remote care coordination, supporting medically complex and cognitively impaired patients with a strong emphasis on:

• Dementia evaluation and management.
• Chronic disease management.
• Goals of care discussions.
• Palliative-focused treatment planning.
• Medication reconciliation.
• Care coordination with families and facility staff.

Providers will participate in interdisciplinary team meetings, quality initiatives, and care conferences with facilities and outpatient teams.

Schedule:

• 4–5 days per week.
• Combination of:
  • In-office clinic days.
  • Assisted living / memory care visits.
  • Remote documentation or telehealth visits.

Primary service area includes:

• North Phoenix.
• Anthem.
• West Valley.
• Scottsdale.
  

Required Licensure / Certifications:

• Active Arizona License as:
  • Physician Assistant or.
  • Nurse Practitioner.
• Board Certification (Family, Internal Medicine, Adult-Gerontology preferred).
• Current BLS Certification.
• DEA License.
  

Personal Qualifications:

• Compassionate and patient-focused.
• Comfortable managing geriatric and dementia populations.
• Strong communication skills with families and caregivers.
• Self-motivated with ability to work both independently and within a team.
• Interest in palliative or supportive care models.
